A Federally Qualified Health Center (FQHC) and a Community Health Center (CHC) offering a full range of primary care medical services and general dental services at affordable rates. Services include: Adolescent Health, Pediatrics, Adult Medicine, Family planning, Geriatric, Gynecology, Women’s Health including the Show Me Healthy Women Program, Diabetic Management, Immunization, Laboratory, Mental Health, Nutrition Counseling, Diabetic Nurse Educator, Foot Care Clinic, Drug Screenings, Well Child services, X-ray, and referral services. Also offers a pharmaceutical assistance program for patients of the clinic who have no insurance and are unable to afford their medicines. Accepts insurance and offers a sliding fee scale. Mental health services provided by an LCSW and accepts insurance, Medicaid, and offers a sliding fee scale. Mental health services can be provided to individuals, couples, and families.
