Provides transportation to medical appointments for those receiving Medicaid for the aged, blind and disabled. Rider must call 3 days in advance for an appointment. The following information will be needed when calling to schedule a ride:
patient’s name, date of birth, address, phone number and the MO HealthNet ID number; name, address and phone number of medical provider; date and time of appointment; medical reason for the transportation request and any special needs of the patient such as a wheelchair or cane.
